The CDC (Centers for Disease Control and Prevention) has formulated a quiz to help individuals 11 years old and up figure out what vaccines they need to have based on factors such as gender, age, medical history, and potential risk.

Go to following CDC website to take this quiz and determine what vaccines you need at this stage of your life. After you receive the results from the quiz, discuss these results with your primary care physician and your parents to ensure you are receiving all the vaccines that you need.

If you don't have a doctor and live in northern New Jersey, you can call the Adolescent/Young Adult Center for Health at 973-971-6475 for an appointment or contact your local teen health center. You can also contact your insurance company for a list of in-network primary care physicians.
